Division:
6 weeks = $115.00
5 weeks = $105.00
4 weeks = $ 95.00
3 weeks = $ 85.00
2 weeks = $ 60.00
1 week = $ 40.00
Red -
White - Blue Divisions:
6 weeks = $165.00
5 weeks = $150.00
4 weeks = $140.00
3 weeks = $130.00
2 weeks = $100.00
1 week = $ 75.00
Indicate
with an "x" the number of weeks.
Please note: If player (s) cannot
attend 2 days in any particular week then any combination of
2 DATES = 1 WEEK
Be sure to indicate the dates on the
schedule provided.
If registering more than one player at a time - Enter the number of
players in applicable week.
Multiply the amount by the amount by this
number for the
Registration Total.
